- Care Coordinators make it possible for seniors with physical or cognitive disabilities to remain at home and live as independently as possible
- Care Coordinators assess needs, determine eligibility for programs and benefits, and secure and monitor services in the home and community.
- Southeast Senior Services has care coordination programs in Ketchikan and Juneau , which also serve the rural communities of Angoon, Hoonah, Metlakatla, and Yakutat.

In-Home Services

- In-Home Services (such as respite, chore services and personal care assistants) help elders to remain in their own homes, living independently.
- Respite offers caregivers an opportunity to rest, relax, run errands or work.
- Chore services are available for elders who are no longer able to maintain their households.
- Personal Care Assistants assist elders with bathing, grooming and dressing.

Senior Information/Caregiver Resource Center

- The Information and Referral Specialist in the Senior Information/Caregiver Resource Center provides information and referrals to seniors and their caregivers on services available in their community, whether their community is in Southeast Alaska or in another state.
- Long-distance caregivers may receive information as well.
- The office is also open to walk-in caregivers who may need to talk through their situation and receive support before receiving pertinent information.

Service Coordination

- Mountain View housing complex has a service coordinator who is available to provide information, to intervene in inter-resident conflicts, to assist with anything from paperwork to shopping, and to arrange educational and social activities to enrich the lives of the residents.
- The Service Coordinator also monitors an Alaska Housing funded chore service program in the building.

Senior Counseling

- Seniors and their caregivers, 55 and older, are eligible to receive free counseling services for depression and other mental health issues through the Senior Counseling program.

Rural Care Advocates

- Rural Care Advocates and Care Coordinators provide information, assistance in obtaining needed services and support to caregivers of elders in Metlakatla, Hoonah, Angoon and Yakutat.

The Bridge Adult Day Program

This program is for:
- Elders who can no longer safely spend the day alone at home while their caregivers work
- Elders who are isolated and need social and mental stimulation.
- Elders who want to make new friends and have fun!
- Elders discover an uplifted, relaxed, nurturing environment at the Bridge. Seniors benefit from activities, outings and special interest programs designed to exercise mind and body.
